What it is

Pattern hair loss is driven by DHT, a more potent derivative of testosterone that miniaturises sensitive follicles. Finasteride blocks the enzyme that makes it. Where minoxidil works on the follicle, this works on the cause — which is why the two are often used together rather than instead of each other. Compounded, and not FDA-approved.

How it is used

One tablet daily, at any time. It takes three to six months before a change is fair to judge, and a year for the full picture. Like minoxidil it is ongoing: stopping returns you to the original trajectory within about a year.

What to discuss with your provider

Tell your provider about any liver disease, depression or mood disorder, prostate concerns or a planned PSA test — finasteride roughly halves PSA readings and your provider needs to know so results are interpreted correctly. Mention any plans for conception.
